WAGE RATE: $41.68 – $53.28

Reporting to the Manager, Diabetes Grey Bruce Program, the Registered Dietitian works in a team environment to assess, plan, develop, implement and evaluate individual counselling and group education programs for people with pre diabetes, diabetes or at high-risk of developing diabetes. Diabetes education is patient-centred, reflects the principles of chronic disease self-management, and incorporates evidence-based standards of practice as identified in current Diabetes Canada Clinical Practice Guidelines for the Prevention and Management of Diabetes in Canada and the Standards for Diabetes Education in Canada. The Registered Dietitian acts as a consultant and resource for other health care professionals with regards to nutritional care and diabetes education and management. This position works in collaboration with same to build capacity and expertise for diabetes prevention and management in various community, long term care and hospital settings. The Registered Dietitian is also responsible for the provision of nutritional care for outpatients in the Oncology Clinic in compliance with professional standards and evidence based practice guidelines. This includes nutrition assessment, care planning, monitoring, education and counselling of patients attending the Oncology Clinic. The Registered Dietitian works within their scope of professional practice in an inter-professional team environment to coordinate all phases of nutrition care.

QUALIFICATIONS

  • Bachelor’s Degree in Human Nutrition or equivalent
  • Graduate from an accredited Internship/Masters Practicum Program
  • General member with the College of Dietitians of Ontario
  • Recent clinical experience (within the last 2 years) in Diabetes Education
  • Certificate in Diabetes Education and/or Principles of Adult Education
  • Certified Diabetes Educator or willingness to obtain within 2 years of employment and maintain certification
  • Able to support intensive insulin therapy (multiple daily insulin injections)
  • Willingness to become certified in insulin management
  • Certified Insulin Pump trainer or recent experience with Insulin Pumps would be an asset
  • Willingness and ability to supervise dietetic interns
  • Experience in providing individual and group instruction to patients, community members and health professionals
  • Excellent communication and interpersonal skills
  • Knowledge and experience utilizing the principles of adult learning and chronic disease self-management
  • Experience working within/or awareness of Aboriginal culture
  • Ability to function effectively in a team environment and as an independent practitioner
  • Knowledge of computer skills to include Word, Excel and PowerPoint applications
  • Commitment to continuous learning
  • Valid Class G Driver’s license and access to a reliable insurable vehicle
  • Ability to travel to venues outside of assigned site
  • Availability to work all shifts, including weekends and available for multi-site coverage when required
  • Recent satisfactory performance and attendance record
