WAGE RATE: $43.19 – $60.83

Reporting to the Outpatient Mental Health and Addictions Manager, the Nurse Coordinator is responsible for the day to day operations for health care services of designated Mental Health & Addiction outpatient services including the relevant intake processes. The Nurse Coordinator provides front line direction to assigned Mental Health and Addictions outpatient staff and its routine functions; oversees the scheduling of referral intakes of outpatient programs; manages the organization of clinical information and co-ordinates between community clinics and outpatient follow up. The Nurse Coordinator liaises with other hospital departments and community partners to ensure good working relationships and patient flow. The Co-ordinator also works with clients and their families to co-ordinate care and resolve problems. Successful applicant must have an exceptional functional knowledge of the Mental Health Act and Health Care Consent Act.

Skills/Abilities/Qualifications

Required:

  • Registration with the Ontario College of Nurses as a Registered Nurse
  • CNA Certification in Psychiatry and Mental Health Nursing or willingness to obtain
  • 5 years recent experience working with individuals with significant mental health issues in an outpatient setting
  • Current training in CPI and GPA
  • Valid CPR certification
  • Excellent knowledge of the MHA, HCCA and SDM legislation
  • Experience in program evaluation
  • Demonstrated leadership skills in a multidisciplinary team setting
  • Knowledge of Recovery Principles
  • Knowledge of current psychiatric medications
  • Psycho/social and mental status assessment skills
  • Experience in medication delivery, administration and monitoring
  • Ability to provide a wide range of counselling, support, advocacy and education services to clients, their family members and others of significance in the client’s life
  • Ability to communicate positively and professionally with family members, community providers and physicians
  • Knowledge of and experience in Cognitive Behavior Therapy
  • Knowledge of Mental Health Act and familiarity with DSM
  • Access to reliable transportation
  • Possession of a valid Ontario Class “G” Driver’s License and personal vehicle.
  • Ability to work co-operatively in a multidisciplinary team setting
  • Knowledge of psychosocial rehab model
  • Recent satisfactory performance and attendance records

Preferred

  • Baccalaureate degree in nursing
